APP - House Calls Program

UChicago Medicine AdventHealth Bolingbrook, GlenOaks, Hinsdale and La Grange hospitals are affiliated with UChicago Medicine, one of the top academic medical centers in the country. We are also part of AdventHealth, a unified health system with over 50 hospital campuses and hundreds of care sites in diverse markets throughout 9 states.

UChicago Medicine AdventHealth is hiring for their Community House Calls program.  This is a growing program which is so needed in our communities. This program serves our homebound patients, giving them access to primary care in the comfort of their homes.

• Experienced APP's preferred, but new grads welcome to apply
• Weekly Monday - Friday schedule
• No call requirement
• Patient visits in assigned territory
• Outpatient care in patients homes
• Resources available for labwork, imaging, procedures, etc.
• Working collaboratively with Physicians, Social Workers and Palliative Care

EDUCATION AND EXPERIENCE REQUIRED:

• Master's degree of Nursing or Physician Assistant Studies required.

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

• BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date. Credentials are verified by local staff credentialing office.
• Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Credentials are verified by local staff credentialing office
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration.
