UChicago Medicine AdventHealth Medical Group has an excellent Family Medicine APP opportunity in our multi-specialty clinic in Carol Stream. 

• APP with 2 or more years of experience practicing, required
• Inherit large patient panel; accepting new patients
• Full-time clinic hours; 36 patient facing hours per week
• 1:4 call
• Patient friendly hours include: 1 evening per week and 1 Saturday morning per month
• Other specialty practices on site
• Lab and Xray on site

• Annual salary: $105K - $118K

REQUIRED QUALIFICATIONS:

Education/Training

• Graduate of an accredited school of nursing and nurse practitioner program
• Masters degree in Nursing or Physician Assistant Studies
• Current Illinois license in good standing
• Certified as a nurse practitioner or physician assistant by an approved certifying organization.
• Prescriptive authority, Illinois Controlled Substance, DEA certification
• Current CPR certification
